Complaint Investigation Report
S2 reported that staff are still awake when C1 arrives from work late. S2 stated that C1 sleeps at the facility everyday.
Regarding the allegation that f acility staff was not available on-call, the following was revealed: During the course of the interviews with clients, C1 reported that staff were not available when he calls. During the initial visit LPA reviewed select telephone calls received by S1 dated April 01, 2025, to May 15, 2025. The calls received by S1 were from C1. Per telephone recent calls the calls from C1 were answer and/or S1 returned the call. During the course of the interviews with staff, S1 reported that when C1 called that he would answer the calls and open the front door right away. S2 reported that S1 always answers the calls from C1. During the interviews AD stated that C1 has not complained to him and reported that C1 also has his main number in case of an emergency. AD stated that he never received a call from C1.
Based on the information gathered during the investigation and review of documents obtained, LPA is unable to ascertain if the allegations occurred as reported due to conflicting information. Although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove or refute the alleged violations occurred; therefore, these allegations are deemed UNSUBSTANTIATED.
For today’s visit, there were no citations issued per Title 22, Division 6 of the California Code of Regulations.
LPA conducted an exit interview with facility representative, and a copy of this report was provided to the facility.
