Facility Evaluation Report
On October 28, 2025, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced required visit to the facility for the purpose of conducting the required annual inspection. LPA explained reason for the visit and was greeted and granted entry by Administrator (AD) Rey Inarin. For this visit, there are two staff members on duty, both of which are background cleared and associated.
LPA observed the Administrator's Certificate for Rey Inarin, which expires on November 28, 2025.
LPA toured the interior and exterior portions of the facility with Rey Inarin. The facility is a single level structure and is licensed for six ambulatory clients. For this visit, there are a total of four clients in care.
There are a total of five bedrooms, four are private client bedrooms, and one is a staff bedroom. LPA toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detectors were tested and operational. The smoke alarm is hard wired and separate from the carbon monoxide detector. There are a total of two bathrooms for clients and staff. Bathrooms were observed to be in good repair and toilets were operational. Water temperature in bathrooms were measured to be between 113.3 and 114.2 degrees Fahrenheit.
Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked underneath the kitchen sink.
