Facility Evaluation Report

Licensing Program Analyst (LPA) Michael Tea attempted to conduct an unannounced annual inspection. Upon arrival there is no one at the facility and is up for lease, LPA contacted phone number on file and left a message regarding visit to the facility. No response. There were no clients observed at the facility. Windows are covered up. The door has a lot of cobwebs and dust and appears to be vacated for a long time. There is a sign up on the window "Office Space For Lease."

This report will be read and reviewed with administrator once LPA reaches the administrator.
