The inspector’s account

On December 11, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Licensee (LI) Jessica Garcia and Administrator (AD) Aisa Guiling were notified via telephone and later arrived to assist with the inspection. LPA observed that Aisa Guiling has a valid Administrator certificate which expires on April 8, 2026.

The facility is an Adult Residential Facility for Persons with Special Health Care Needs (ARFPSHN) licensed for five non-ambulatory clients, of which five may be bedridden. The facility is a single story home with five client bedrooms, two client bathrooms, a living room, a dining room, a kitchen, a staff office, and an attached two car garage. LPA accompanied by the LI and the AD conducted a tour of the interior portion of the facility. On today's visit, LPA observed two clients in care. LPA inspected all five client bedrooms, and they were observed to be free of any hazards. All client beds had clean linens and blankets. LPA observed additional linens and blankets are stored in each client's private closet. LPA inspected the two shared client bathrooms. Client bathrooms are clean. Faucets and toilets were operational. Hot water temperature measured between 108.6 and 112.6 degrees Fahrenheit.

LPA observed the kitchen has a two day perishable and seven day nonperishable food supply on hand. LPA observed kitchen appliances to be clean and operational. LPA observed kitchen knives are stored in a locked kitchen cabinet. LPA observed chemicals and toxins to be stored in a locked kitchen cabinet under the sink. A fire extinguisher is located in the kitchen and in the client hallway. Fire extinguishers were observed the be charged and serviced as of January 13, 2025. LPA tested the wired smoke detectors/carbon monoxide detectors which tested operational. CONTINUED ON LIC809-C
