Facility Evaluation Report
On August 18, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct a Case Management Incident inspection. LPA is following up on a self reported incident report that was submitted to the Orange County Regional Office on August 6, 2025. The incident report describes Staff #1 (S1) yelling and using physical force on Client #1 (C1). LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Assistant Administrator (AA) German Navarrete was notified via telephone and later arrived to assist with the inspection.
LPA, accompanied by staff, conducted a tour of the physical plant of the facility. On today's visit, LPA observed five clients in care and two staff present. LPA observed clients returning from their respective Day Programs and watching TV in the living room. LPA observed the facility to be clear of any hazards or obstructions. No health or safety concerns were observed. LPA reviewed and obtained pertinent documents to the incident including client records, staff records, and staff training. LPA additionally conducted two client interviews and conducted three staff interviews.
Based on the information gathered during the visit, deficiencies are being cited on the attached LIC809-D. An exit interview was conducted with Assistant Administrator (AA) German Navarrete. A copy of the report and Appeal Rights were provided.
