Facility Evaluation Report

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced visit to conduct the annual required inspection. LPA was greeted and granted entry by Administrator/ Licensee Vernell Laurence and explained the reason for the visit. There are no clients present. Licensee indicated the facility has not had admitted clients for approximately two years.

LPA toured the facility and observed a two story home with two bedrooms, two restrooms, living room and dining room on the first floor and five bedrooms with two restrooms on the second floor. LPA observed facility is cluttered and in need of organizing/ cleaning.

Prior to accepting clients, Licensee to organize and remove clutter in facility. Licensee to notify the department when ready to accept clients. The department to inspect facility prior to accepting any new clients.

No citations delivered during today's visit. Exit interview conducted and a copy of this report was left at the facility.
