Facility Evaluation Report

An Informal meeting was conducted on this date in Orange County Adult and Senior Regional Office. The purpose of this informal conference was to discuss expectations of the facility and Community Care Licensing staff.

Present in this meeting were Regional Manager Marina Stanic, Licensing Program Manager Lourdes Montoya, Licensing Program Analyst Jenifer Tirre, Licensee Vernell Laurence and Staff Member Patricia Ingram.

The meeting process was explained to the Licensee. Also discussed was the expectations of Licensee, status of current license, process of change of ownership, new application, lease back agreement and control of property.

Licensee agreed that in a months time by October 17, 2025 they will contact department regarding their plan of what to do with license of facility. Licensee agreed to meet back with Department on October 17, 2025.

An exit interview was conducted and copy of report was given to Licensee Laurence.
