Facility Evaluation Report

This an announced inspection is being conducted by Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. for the purpose of conducting a closure visit. LPA met with Licensee Diane Hall and Ron Salda. LPA explained the purpose of the visit.

On September 2, 2025, Licensee emailed Community Care Licensing (CCL) requesting the facility closure. During today's visit the Licensee surrender the License.

LPA and facility representative toured the facility and observed the following: There are no clients in care. The facility is not providing care and/or supervision. LPA observed the facility to be clean and sanitary.

LPA informed the facility representative that LPA will close the facility and that a closure letter would be mail.

There are no immediately health and safety concerns, as the facility has a census of zero.

An exit interview was conducted and a copy of this report was provided at the time of exit.
