Facility Evaluation Report
On October 10, 2025, Licensing Program Analyst (LPA) Alvaro Ramirez, Jr. made an unannounced visit to the facility to conduct the required annual inspection. LPA met with Licensee Diane Hall and discussed the purpose of the inspection.
LPA and Hall toured the facility and observed the following: There are no clients in care. The facility is not providing care and/or supervision. The facility is being used as a residential home.
Per Hall, the last client left the facility late 2023 or early 2024.
There are no immediate health and safety concerns, as the facility has a census of zero.
Based on the observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted and a copy of this report was provided at the time of exit.
