The inspector’s account

On this day, Licensing Program Analyst (LPA) Kimberly Lyman made an unannounced visit to conduct a required annual visit. LPA was greeted and granted entry into the facility by Administrator (AD) Zoila Flores and explained the reason for the visit. Facility is licensed for 6 clients of which 2 may be non-ambulatory. AD has a pending Administrator Certificate renewal as noted on the department website.

LPA along with AD toured the facility at 8:45 AM. LPA toured the physical plant, checked food service, and facility documentation. The home is a 2 story building consisting of 3 client bedrooms, staff room and 2 bathrooms upstairs and 3 staff rooms, 2 bathrooms, 2 kitchens, living room, dining room and garage downstairs. Client bedrooms had the required furniture, bed linens and closet/drawer space to accommodate each client comfortably. Client bathrooms were checked. Toilets and water faucets worked properly, grab bars were secure and shower was free of mold/mildew. Water temperature measured between 111.2 degrees F and 117.5 degrees F in all bathrooms. Client bath towels, toiletries and personal hygiene supplies were adequately stocked. Common areas were clean and clear of hazards. LPA toured the double kitchens and observed sharps secured during today's visit. Perishable and non-perishable food supply was checked and adequately stocked at time of visit. Kitchen appliances were operational during today's visit. Smoke detectors tested operational during today's visit. Fire extinguishers were fully charged. LPA reviewed the infection control plan which is thorough and complete. Facility conducts emergency drills with the last drill conducted on 10/23/2025. Outside grounds were toured. Exit gates are operational. Walkways around the home were clear of hazards. There is ample shaded outdoor seating for clients. First aid kit contained all required items including tweezers, scissors and thermometer. LPA observed the ample emergency food and water supply.
