Facility Evaluation Report
On December 2, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Program Director (PD) Maria Vega was present and assisted on today's visit.
The facility is an Adult Day Program (ADP) licensed for fifty non-ambulatory clients. The facility is a commercial business complex which consist of three buildings. The first building consist of a reception area, four office areas, a staff break room, a storage room, and a computer room. The second building consist of a multi-purpose room, an open activity area, an arts and crafts room, a storage area, a quiet room, and three restrooms. The third building consist of a kitchen/dining area, a lounge, three storage rooms, five staff office areas, a conference room, two personal care rooms, and two restrooms. On today's visit, the facility has a census of sixty eight clients, however, only fifteen clients were physically present during the visit since the other clients were in community outings. LPA, accompanied by the PD, conducted a tour of the physical plant. LPA inspected the kitchen and observed it to be free of any hazards. The facility does not provide food to the clients but offers a refrigerator for clients to store their food and appliances for clients to heat their food. LPA observed kitchen appliances such as the refrigerator, microwave, and air fryer to be clean and operational. Kitchen knives and sharps are storage inaccessible to clients. LPA inspected each common area and in each of the three buildings and observed them to be free of any hazards or obstructions. LPA inspected the restrooms and observed them to be clean and free of any hazards. Faucets and toilets were operational. LPA observed chemicals and toxins to be stored in locked cabinets located near the restrooms in the third buildings. CONTINUED ON LIC809-C
