Facility Evaluation Report
Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility for the purpose of conducting the annual inspection. LPA was greeted and granted entry by care giving staff after explaining the purpose for the visit. Administrator (AD) Claudia Olteanu was notified via telephone and later arrived to assist with the inspection. LPA observed that Administrator Claudia Olteanu has a valid Administrator certificate which expires on June, 13, 2026.
The facility is a Residential Care Facility for the Elderly (RCFE) licensed for six non-ambulatory residents, of which six may be bedridden, and has a hospice waiver for two. The facility is a single story home with six resident bedrooms, one of which is shared and one of which is vacant, four shared resident bathrooms, three staff rooms, a living room, a dining room, a kitchen, a staff office, a laundry room, a storage room, an attached two car garage, and an attached ADU with three staff rooms, a staff kitchen, and a staff living room. LPA, accompanied by the AD, conducted a tour of the interior portion of the facility. On today's visit, LPA observed five residents in care, one resident was away at her Day Program per AD, and three care giving staff present. LPA observed residents relaxing in their respective bedrooms and in the living room. LPA observed the See Something, Say Something poster (PUB 475) mounted by the entryway of the facility. LPA inspected the six resident bedrooms and they were observed to be free of hazards. Resident bedrooms had the required furnishings of a bed, a chair, a chest of drawers, and a lamp. Resident linens and blankets were clean. LPA observed additional linens to be stored in the storage room. LPA inspected the four resident bathrooms and they were observed to be clean. Resident bathrooms were equipped with grab bars and non skid floor mats. Faucets and toilets were operational. Hot water temperature measured between 112.4 and 112.6 degrees Fahrenheit. LPA observed staff rooms are kept locked and inaccessible to residents in care.
