The inspector’s account

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Mike Yamashiro Director of Operations (DO) and discussed the purpose of the visit. The facility currently has five clients in care.

The facility is a two story home with three client bedrooms, three bathrooms, a medication office, a group room, a clinical office, living room, kitchen, dining room and attached two car garage. The facility appears clean, safe and sanitary. LPA toured the facility and observed all client bedrooms had the required components and furnishings. LPA observed the facility kitchen to be clean and free of vermin. A two day perishable and seven day nonperishable food supply was on hand. LPA observed all appliances to be operational. LPA observed the toxins and chemicals to be in a locked cabinet located by the washer and dryer and made inaccessible. LPA observed the garage to be locked and made inaccessible to clients. The garage stores extra facility supplies including fridges and freezers as well as more pantry items. LPA observed the emergency food and water supply to be located in the garage as well. LPA observed clean linens in the closet by the front door for client use. LPA observed an evacuation chair at the top of the stairs. The medication room was locked and made inaccessible to clients. LPA observed the centrally stored medication to be in a locked closet located in the locked medication room. LPA observed the first aid kit to have all the required components. LPA observed the knives to be in a locked closet located in the locked medication room. LPA observed the bathrooms to have toilet paper, paper towels and textured shower flooring. The water was tested to be between 105.1-109.9 degrees Fahrenheit. LPA observed fire extinguishers throughout the facility charged and with a service date of February 10, 2026. The backyard has a shaded seating area for client use and was free of debris and obstructions. DO tested the carbon monoxide and smoke detectors and they were found to be operational.
