Facility Evaluation Report

Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to conduct a Plan of Correction (POC) visit for deficiencies issued during the required annual inspection on June 17, 2026. LPA was greeted and granted entry by staff. LPA met with Direct Support Professional (DSP) Edgar Barrido and DSP called Administrator Myrna Calub and discussed the purpose of the visit.

Regarding the Type B Citation 80066(a)(10) . LPA reviewed a health screening with record of a TB test dated July 14, 2026, for 1 of 1 staff. Thus, the citation was fulfilled.

Regarding the Type B Citation 80069(c)(1) . LPA reviewed a TB test dated June 26, 2026, for 1 of 1 clients in care. Thus, the citation was fulfilled.

Based on record review no citations were noted per Title 22 Division 6 of the California Code of Regulations. All citations issued during the required annual inspection on June 17, 2026, have been cleared. An exit interview was conducted and a copy of this report along with clearance letter were left at the facility.
