Facility Evaluation Report
Licensing Program Analyst (LPA) Hanna Gough made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry by staff. LPA met with Direct Support Professional (DSP) Edgar Barrido and discussed the purpose of the visit. DSP contacted Administrator (AD) Myrna Calub and informed them of LPAs presence at the facility and they informed DSP that they are available by phone. The facility currently has four clients in care of which all were out at day program.
The facility is a one-story home with four client bedrooms, one staff room, bathrooms, living room, kitchen, dining room and two car garage. LPA observed all client bedrooms had the required components and furnishings. LPA observed a clean supply of linens in the cabinet in the hall by the client bedrooms. LPA observed the bathrooms to have toilet paper, soap and paper towels. LPA tested the water to be between 110.4 and 116.9 degrees Fahrenheit. LPA observed the kitchen to be clean and free of vermin. LPA observed a two day perishable and seven day non perishable food supply on hand. LPA observed all appliances to be operational. The toxins and chemicals are locked under the kitchen sink and in a cabinet next to the kitchen and made inaccessible. The knives and sharps are locked in a kitchen drawer and made inaccessible. LPA observed a fire extinguisher in the dining room charged and with a service date of July 28, 2025. The centrally stored medication is in a locked cabinet in the dining room and made inaccessible. LPA observed the first aid kit to have all the required components. The staff room was observed to be locked and made inaccessible. DSP tested the fire alarms and smoke detectors and they were found to be operational. The front yard has a shaded seating area for client use. The garage has extra facility food storage in extra fridges and freezers along with a food pantry. LPA observed the emergency food and water supply was stored in the garage as well. LPA observed the garage to have couches and a tv. DSP informed LPA that they use the garage as an additional communal space for entertainment. The backyard was observed to have three sheds of which all are used for storage. The backyard was free of debris and obstructions.
