Facility Evaluation Report
On May 6, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct the required annual inspection. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Chief Operating Officer (COO) William Kelsey
and Chief Strategy Officer (CSO) Reese Perez were present and assisted on today's visit.
The facility is an Adult Day Program (ADP) licensed for one hundred and twenty one ambulatory clients. The
facility's operates Monday through Friday from 8 AM to 2 PM. The facility is a two story commercial building. The first floor consist of two staff offices, one staff room, a lobby area, a kitchen area, thirteen client activity rooms, two storage rooms, and two client bathrooms. The second story consist of five client activity rooms, a
kitchen area, two storage rooms, and two client bathrooms. LPA, accompanied by the COO and CSO, conducted a tour of the physical plant. LPA observed the first and second floor to be free of any hazards or obstructions. LPA inspected all the client activity rooms on the first and second floor and observed them to be free of hazards. LPA observed the facility offers activities to clients such as exercise, sensory activities, art therapy, music therapy, fine motor activities, and a computer lab. LPA observed staff offices and rooms to be kept locked and inaccessible to clients in care. LPA observed the storage rooms on the first and second floor to be kept locked and inaccessible to clients in care. LPA observed the locked storage room to contain the facility's chemicals and toxins. LPA inspected the client bathrooms on the first and second floor and observed them be clean. Faucets and toilets were operational. LPA inspected the kitchen areas on the first and second floor and observed it to be clear of hazards. The facility does not provide food to clients but do offer kitchen appliances such as refrigerators, microwaves, and toaster ovens. LPA observed all kitchen appliances to be clean and operational.
