Facility Evaluation Report
On May 14, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to conduct a Case Management - Incident inspection. LPA is following up on a self-reported incident report that received by the Orange County Regional Office on May 7, 2025 involving a food incident with Client #1. LPA was greeted and granted entry into the facility by staff after explaining the purpose for today's visit. Administrators William Kelsey and Reese Perez were present and assisted LPA with the inspection.
On today's visit, LPA conducted a tour of the physical plant to conduct a health and safety check. LPA observed clients to be in good health and in good spirits. LPA observed clients preparing to leave the facility for a scheduled outing. LPA reviewed and obtained relevant documents to the incident which include the staff roster, client roster, C1's face sheet, C1's Development and Evaluation Report, C1's Physician's Report, and C1's Individual Program Plan.
LPA conducted six staff interviews with staff that were present at the time of the incident.
Due to insufficient information available at this time and further interviews that need to be conducted, this incident needs further investigation. An exit interview was conducted with Administrators William Kelsey and Reese Perez. A copy of the report was provided.
