Facility Evaluation Report

On this day, Licensing Program Analyst (LPA) Kevin Saborit-Guasch made an unannounced visit to the facility for the purpose of following up on deficiencies cited during the required annual inspection visi conducted on December 12, 2025. LPA was greeted and granted entry by facility licensee Teresita Gutierrez after introducing himself and stating the purpose of the visit.

During the present visit, LPA verified that the water fixture off of the main living room had been properly affixed to the wall and was no longer loose. Repairs have been completed, citation is cleared on this day. Water temperature was verified at two faucets used for personal grooming as well as at one of the showers. Water temperature was within acceptable range at all three locations. Deficiency cleared.

Plans of corrections for other deficiencies are still pending at this time. Licensee stated administrator Romualdo Amante would be providing proof of correction including training certificates for required annual staff training shortly.

No additional deficiencies issued at this time. An exit interview was conducted and a copy of this report along with two clearance letters were provided to a facility representative.
