Facility Evaluation Report

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the facility closure visit. LPA met with Program Director Catherine Aquino. The Program Director reported the facility has no clients at this time and is closing. LPA and the Program Director toured the facility. LPA observed the facility has no personal belongings of clients. LPA verified no one is residing at the facility. The Program Director surrendered the facility license. LPA explained the facility closing process to the Program Director who stated they understood. LPA will close the facility in the system and sent the Licensee the closure letter. The facility is now closed. An exit interview was conducted and a copy of the report provided.
