Staff did not reveal the facility license number in a advertisement.

Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the required 10-day visit to begin the investigation into the allegation listed above. LPA met with staff and explained the reason for the visit. LPA and staff toured the facility. The investigation into the allegation, staff did not reveal the facility license number in a advertisement, revealed the following. A review of the website;

https://serenebh.com , shows the website does not list the State of California Department of Social Services and does not list the license number of the facility. LPA confirmed with the staff that the website is the website for the Licensee and the facility. Based on information gathered, the preponderance of evidence standard has been met, therefore, the allegation is found to be substantiated. Violations are being cited per California Code of Regulations, Title 22, Division 6, Chapter 2. An exit interview was conducted and a copy of this report along with citation and Appeal Rights (LIC 9058 01/16) was provided.
