Facility Evaluation Report
Licensing Program Analyst (LPA) Joseph Alejandre made an unannounced visit to conduct the pre-licensing inspection. LPA met with Program Director Brittany Stipes. An initial application to operate an Adult Day Program for 50 ambulatory clients was submitted to Community Care Licensing (CCL) on August 20, 2024. The Program Director reported most of the programs provided are community based and the facility does not have an outdoor activity area. Facility hours are Monday through Friday 7:30am to 3:30pm.
Structure . The facility is a two story building located in a business office complex. Facility has 5 rooms on the second floor and 3 rooms on the first floor. The second floor is for facility operations and will not be used for client activities. The first floor has a large open space with a reception area and 3 activity rooms. The rooms will be used for functional skills, art therapy, vocational skills, sensory room, music therapy and computer activities. LPA observed a fenced area with fitness equipment on the first floor. LPA observed games and puzzles stored in cabinets in the activity rooms. LPA observed all fire extinguishers on the first and second floor are fully charged. Facility phone number is 949-234-8250.
Furniture , LPA observed numerous tables and chairs for clients to sit and do activities at. Bathrooms . Facility has 2 restrooms on the first floor. Both restrooms are clean and operational. Water Temperature . LPA measured the hot water at 108.5 degrees Fahrenheit. Emergency Phone Numbers & Exit Plan . Posted at the reception area, stairway and on the second floor.. Appliances . None, facility has no appliances. Food Service . None, clients will bring their own food & snacks. Drinking Water. Available & located inside the building for clients use. Toxins and cleaning supplies . Locked in the storage closet inaccessible to clients. Clients & Staff Files . Will be stored electronically. Medications . Facility will not handle medication. First-Aid Kit & Book. There is a first aid kit on each floor. LPA observed both first aid kits have all the required elements. Reading Material, Games, Equipment & Materials . There are board games, magazines, books, televisions, music area, and arts and crafts for clients stored in the activity rooms.
