Facility Evaluation Report
On July 27, 2026, Licensing Program Analysts (LPAs) Brandon Lopez, Taylor Simerly, and Tran Nguyen made an unannounced visit to the facility to conduct the required annual inspection. LPAs were greeted and granted entry into the facility by staff after explaining the purpose for the visit. Program Director Bobby Santangelo was notified via telephone but was unable to assist with today's inspection.
The facility is a Social Rehabilitation Facility (SRF) - Transitional Residential Treatment Program licensed for six ambulatory clients. The facility is a single story home with three client bedrooms, two bathrooms, a living room, a dining room, a kitchen, one staff office, and an attached two car garage. There is also a therapy room located in an additional dwelling unit in the backyard. LPAs, accompanied by a staff, conducted a tour of the interior portions of the facility. On today's visit, LPAs observed two clients in care and three staff present. LPAs observed the clients relaxing in the backyard. LPAs inspected the three client bedrooms and observed at the them to be free of hazards. LPAs observed client bedrooms to have the required furnishings of a bed, a chair, a chest of drawers, and a lamp. LPAs observed client beds to have clean linens and blankets. LPAs observed additional linens to be stored in a hallway closet. LPAs inspected the client bathrooms and observed them to be clean. Faucets and toilets were operational. Hot water temperature measured 113.7 degrees Fahrenheit.
LPAs observed the kitchen has a two day perishable and a seven day non-perishable food supply on hand. LPAs observed kitchen appliances to be clean and operational. LPA observed the six burner gas stove lights unassisted. LPAs observed the facility has a total of multiple fire extinguishers that are mounted on the walls across the facility including in the kitchen, and in the garage.
