Facility Evaluation Report
Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA explained the reason for the visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted facility administrator (AD) Niloufar Nekou about the visit. For this visit, there are six staff members on duty, all of which are background cleared and associated.
LPA toured the interior and exterior portions of the facility with Niloufar Nekou. The facility is a two-level structure and is licensed for six ambulatory clients from the ages of 18 to 65. For this visit, there are a total of six clients in care.
There are a total of four client bedrooms, two of which are private client rooms, and two are shared client rooms. LPA toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detectors were hardwired and auditory exit alarms were tested and operational. There are a total of two bathrooms and one half bath, two of which are for clients and one is for staff. Bathrooms were observed to be in good repair and toilets were operational. Water temperature in restrooms were measured to be between 107.7 to 109.5 degrees Fahrenheit.
Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked in the nurse's station and made inaccessible to clients in care. Fire extinguisher was charged, mounted and located in the kitchen, patio, second floor. Continued on LIC 809-C.
