Facility Evaluation Report
Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced complaint visit to follow up on an incident report received by the department on 05/29/2026. LPA was greeted and granted entry into the facility and explained the reason for the visit.
Incident report dated 05/15/2026 indicated Resident 1 (R1) had been discovered by an off duty caregiver in the parking lot of Ralph's approximately .2 miles from the facility. Caregiver transported the resident back to the facility with no adverse effects. Physician report dated 03/19/2026 indicates resident is diagnosed with Dementia and has elopement as a behavioral expression. The physician report states the resident is unable to leave the facility unassisted. Staff were unaware the resident had eloped out of the memory care unit.
Based on the observations made during today’s visit, deficiency is being cited per Title 22 Division 6 of the California Code of Regulations. This report was discussed with the Administrator and a copy was provided as well as appeal rights.
