Email correspondence dated 04/03/2026 show additional requested documents were provided to the responsible party including incident report and updated physician report. Based on record review and interviews conducted, the allegations are deemed UNFOUNDED, meaning that the allegation is false, could not have happened and/or is without a reasonable basis. Exit interview conducted and a copy of this report was left at the facility.
