Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced case management visit to deliver an amended report. LPA was greeted and granted entry into the facility and explained the reason for the visit.
During the visit, LPA delivered an amended report originally delivered on 12/23/2025 as well as toured the facility. LPA observed residents either eating lunch or participating in activities. No health or safety concerns noted during visit.
Exit interview conducted and a copy of this report was emailed to Administrator.
