Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced health and safety case management visit to the facility. LPA was greeted and granted entry into the facility and explained the reason for the visit.
During the visit, LPA toured the facility and spoke with residents. Residents were observed relaxing or dining in the facility and appeared clean and well taken care of. Facility appears clean, safe and sanitary. LPA observed ample staffing in the memory care unit. LPA observed no health or safety concerns during the visit today.
Exit interview conducted and a copy of this report was left at the facility.
