Facility Evaluation Report

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced Plan of Correction (POC) visit to follow up on deficiencies cited on 05/12/2025. LPA was greeted and granted entry into the facility and explained the reason for the visit.

Deficiency cited under Title 22 Regulation 87303(e)(2) pertaining to Maintenance and Operation, Water Temperature has NOT been cleared. During the visit, LPA tested water temperature to be between 126 and 132.4 degrees F. Licensee has NOT complied with the POC however, LPA was provided with water temperature log as facility has been taking temperatures and addressing the water temperature issue. Facility ordered a new tank less water heater with permit approved 05/13/2025.

Deficiency cited under Title 22 Regulation 87211(a)(1)(D) pertaining to Reporting Requirements has been cleared. Licensee provided proof of correction.

Licensee has been advised to maintain compliance in all items previously cited.

Exit interview conducted and a copy of this report was left at the facility.
