Facility Evaluation Report

Licensing Program Analyst (LPA) Christian Gutierrez conducted an unannounced annual inspection using the Compliance and Regulatory Enforcement (CARE) tools. LPA met with Ernie Enriquez brother of licensee at approximately 11:40 AM and explained reason for visit. Licensee Maila Enriquez arrived shortly.

Currently there are NO residents residing at this facility.

LPA was accompanied by the Ernie Enriquez and provided a tour of the home. Based on observation, no residents or personal belongings were present at the facility.

No files were reviewed on this date. Administrator's certificate is current and expires in January of 2025.

Based on today's inspection no deficiencies were observed at this time a copy of this report was provided to Ernie Enriquez.
