Facility Evaluation Report
On May 30, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the follow to conduct a Case Management Incident inspection. LPA is following up on an incident report that received by the Orange County Regional Office on May 15, 2025. The incident report was dated March 10, 2025 and described a medication error for Resident #1 (R1). LPA was greeted and granted entry into the facility by staff after explaining the purpose for today's visit. Executive Director (ED) Benjamin Davis was present and assisted LPA with the inspection.
On today's visit, LPA, accompanied by the ED, conducted a tour of the facility. LPA observed the facility to be clear of any obstructions or hazards. LPA observed residents participating in various activities such as painting and reading. LPA additionally reviewed pertinent documents for R1. LPA also conducted interviews with R1 and two staff.
Based on the observations made during today's visit and the information gathered, a deficiency is being cited on the attached 809-D. An exit interview was conducted with Executive Director (ED) Benjamin Davis. A copy of the report and Appeal Rights were provided.
