Facility Evaluation Report

Licensing Program Analyst (LPA) Patricia Velazquez conducted an unannounced visit to Billy's House. LPA Velazquez was allowed entry into the home and met with Administrator Karry Vallejo. During the complaint investigation visit with complaint number: 22-AS-20220908144115 LPA Velazquez observed Staff Nely Guerrero did not appear on the facility's LIC 500 form which is their staff work schedule and did not appear on the Facility Personnel Report Summary. At 1:47 PM LPA Velazquez proceeded to call the Orange Regional Office and LPA was informed that Staff Nely Guerrero was not associated to Billy's House.

Deficiencies cited under California Code of Regulations Title 22, Division 6, Chapter 1 and a Civil Penalty was also issued.

An exit interview was conducted with Administrator Karry Vallejo and a copy of this report along with the appeal rights, and LIC 9098 were provided at the time of this visit.
