Facility Evaluation Report
Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility for the purpose of conducting a follow up Case Management Incident inspection. Please see LIC809 dated 2/5/25 for the details regarding the initial inspection conducted. LPA was greeted and granted entry by Assistant Program Director (APD) Krysten Bailey after explaining the purpose for the visit.
LPA conducted a tour of the physical plant and checked on the health and safety of clients in care. On today's visit, LPA observed thirteen clients in care and six staff present. LPA observed client relaxing in their respective bedrooms and in common areas. LPA observed clients to be clean and in good health. LPA observed the facility to be clean and clear of obstructions and hazards.
Based on the interviews conducted and records reviewed, no deficiences are being cited for the self-report incident that was received by the Orange County Regional Office (OCRO) on 2/4/25. An exit interview was conducted with Assistant Program Director (APD) Krysten Bailey and a copy of the report was provided.
