On this day Licensing Program Analyst (LPA) Fred Arias made an unannounced visit to conduct a case management visit. LPA was greeted and granted entry into the facility by staff and explained the reason for the visit. LPA met with Residential Supervisor (RS) Brianna Graham. LPA amended original LIC9099-A report dated 04/23/2025 and discussed details with RS.
An exit interview was conducted and a copy of this report was left with facility representative.
