LPA interviews with six out of eight clients stated they had no issues with mail delivery. One out of the remaining two clients stated their mail was held and not given timely, sometimes delayed by a day or two. The remaining client did not add anything relevant to the allegation investigated. LPA interviews with five out of five staff stated mail and packages are provided to clients shorty after it arrives, usually during break time the same day. Packages are checked for contraband with the client present.
Regarding the allegation staff did not treat client with dignity and respect, it was reported staff member 1 (S1) engaged in conversations revolving dark humor, ridicule, and blame with a client.
LPA interviews with six out of eight clients stated they were treated well and did not have issues with staff. Two out of eight clients stated S1 was unprofessional and made them feel worse during their time at the facility. LPA interviews with two out of five staff stated clients are treated well and have not witnessed any abuse from staff towards clients. One out of the remaining three staff stated clients have the option of changing staff members they directly work with if the client is not happy. One out of the remaining two staff stated staff are very empathetic to client needs. The remaining staff member did not add anything relevant to the allegation investigated. LPA record review for two staff present revealed they received mandatory reporting requirements training.
Therefore based on client interviews, staff interviews, records observed, and LPA observations, the allegations of staff did not provide client with their mail in a timely manner and allegation staff did not treat client with dignity and respect are therefore deemed unsubstantiated meaning that although the allegations may have happened or are valid, there is not a preponderance of the evidence to prove that the alleged violations occurred.
An exit interview was conducted and a copy of the report was left with the facility representative.
