Facility Evaluation Report
On January 7, 2026, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to the facility to conduct a Case Management - Incident inspection. LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Residential Supervisor (RS) Ryan Reifsnyder was present and assisted on today's visit.
LPA is following up on a self reported incident report that was received by the Orange County Regional Office on December 31, 2025, regarding a medication error with Client #1 (C1). The incident report stated that on December 29, 2025, C1 was given two nicotine patches within a twenty four hour period, when C1 is supposed to have one nicotine patch a day. On today's visit, LPA observed three clients in care. LPA, accompanied by RS, conducted a tour of the physical plant. LPA observed the facility to be clear of any obstructions and hazards. No health or safety concerns were observed. LPA conducted one client interview and one staff interview. LPA additionally reviewed and obtained client records. Interviews conducted confirmed the incident report that was received by the Orange County Regional Office was accurate and that a medication error did occur with C1's medication. LPA also reviewed records which confirmed the medication error occurred.
Based on the information gathered during today's visit, a deficiency is being cited on the attached LIC809-D. An exit interview was conducted with Residential Supervisor Ryan Reifsynder. A copy of the report and Appeal Rights were provided.
