The inspector’s account

On August 29, 2025, Licensing Program Analyst (LPA) Brandon Lopez made an unannounced visit to conduct a Case Management Incident inspection. LPA is following up on a self reported incident report that was received by the Orange County Regional Office on August 22, 2025, regarding an incident with Client #1 (C1). LPA was greeted and granted entry into the facility by staff after explaining the purpose for the visit. Residential Supervisor (RS) Ryan Reifsynder was present and assisted on today's inspection.

On today's visit, LPA observed five clients in care and two staff present. LPA, accompanied by RS, conducted a tour of the physical plant. LPA observed the facility to be clear of any obstructions and hazards. No health or safety concerns were observed. LPA additionally reviewed and obtained client records. LPA also conducted one staff interview.

Based on the observations and information gathered during today's visit, no deficiencies are being cited per Title 22 of the California Code of Regulations. An exit interview was conducted with Residential Supervisor Ryan Reifsynder and a copy of the report was provided.
