Complaint Investigation Report

LPA conducted an additional four staff interviews. Four out of the four staff interviewed denied the allegation and stated that they have never observed, or heard of S1 speaking inappropriately while at the facility. The four staff also denied any other staff speaking inappropriately in the presence of clients.

Based on the information gathered during the investigation, the Department is unable to ascertain if the allegation occurred as reported. Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove or refute the alleged violation occurred; therefore, the allegation is deemed Unsubstantiated. An exit interview was conducted with Operations Supervisor Joey Nazario and a copy of the report was provided at time of visit.
