Facility Evaluation Report

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced case management visit to conduct a closure visit. LPA contacted Director of Quality and Compliance by telephone who arrived at 9:00 AM. LPA was greeted and granted entry into the facility and explained the reason for the visit.

On 09/16/2025, Facility notified the department of intent to surrender the license. Clients were moved to the newly licensed facility at 2351 Azure Newport Beach on 09/13/2025. Clients were informed on 09/05/2025 of the pending license at the new facility and potential to move. Clients residing at the facility voluntarily agreed to move to the new facility. Licensee surrendered the license at time of visit and was provided a closure letter by LPA.

Exit interview conducted and a copy of this report was left at the facility.
