02/21/2023 and conducted by Evaluator Kimberly Lyman

Licensee is making and disseminating misleading statements about the facility.

Licensing Program Analyst (LPA) Kimberly Lyman conducted an attempted visit to the facility. There was no response at the front door and LPA contacted Administrator Greg Rosenbaum who stated Chief Financial Officer (CFO) Max Reznik would be arriving shortly.

Licensing Program Analyst (LPA) Kimberly Lyman conducted an unannounced complaint visit to initiate an investigation into the above allegation. LPA was greeted and granted entry into the facility by CFO Max Reznik and explained the reason for the visit.

During the course of the investigation, LPA interviewed CFO as well as reviewed facility website. Facility website indicates Mental Health Collective facilities are one block from the ocean. Facility is approximately half a mile from the ocean. LPA observed on the facility website pictures of a facility not currently licensed by the department. The preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 6, Chapter 8), are being cited on the attached LIC 9099D. An exit interview was conducted and a copy of this report was provided by email to facility along with appeal rights.
