11/21/2022 and conducted by Evaluator Kimberly Lyman

COMPLAINT CONTROL NUMBER: 22-AS-20221121101337

Licensee is not revealing the facility license number in all advertisements as required.

Licensing Program Analysts (LPAs) Kimberly Lyman and Alvaro Ramirez conducted an unannounced complaint visit to initiate an investigation into the above allegation. LPAs were greeted and granted entry into the facility and explained the reason for the visit. Administrator Greg Rosenbaum arrived during the visit.

During the investigation, LPA reviewed facility website and interviewed Chief Financial Officer Max Reznik. Regarding the allegation that licensee is not revealing the facility license number in all advertisements as required, the investigation revealed the following: LPA observed facility is not posting license number on website.

The preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 6, Chapter 8), are being cited on the attached LIC 9099D. An exit interview was conducted and a copy of this report was provided to facility along with appeal rights.
