The inspector’s account

On July 9, 2026, at 9:00 AM, Licensing Program Analyst (LPA) Edward Kim conducted a case management visit to verify facility closure. LPA Kim met with Licensee Allen Medina to verify facility ceased operation and to obtain the surrendered license. Licensee surrendered the facility license. LPA Kim conducted a walk through with Licensee Medina and verified there are no residents and signs of operation. Licensee submitted the letter of intent to close the facility on July 9, 2026, and the residents’ last day was on June 30, 2026.

The location will no longer function as an Residential Care Facility for the Elderly. Based on the observation and interview, the facility is now closed. The Department will proceed with the closure as of July 1, 2026. LPA Kim informed the above facility representative of the licensing procedure for future facility operation if desired.

An exit interview was conducted and a copy of this report was provided to Licensee Allen Medina.
