Facility Evaluation Report
On March 10, 2026, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA explained reason for visit and was greeted and granted entry by staff on duty. During the visit, staff on duty contacted facility administrator (AD) Marites De Vera about the visit. For this visit, there is one staff member on duty, who is background cleared and associated.
LPA observed the Administrator's Certificate for Marites De Vera, which expires on March 30, 2026.
LPA toured the interior and exterior portions of the facility with staff. The facility is a single level structure and is licensed for four clients between the ages of 18 to 59. The facility is an Adult Residential Facility (ARF) licensed for four ambulatory clients. For this visit, there are a total of three clients in care.
There are a total of four bedrooms, three of which are private client rooms, one is a shared client room and one is a staff bedroom. LPA toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detector were tested and operational. There are a total of two bathrooms, one is for staff and one is for clients. Bathrooms were observed to be in good repair, toilets were operational, and grab bars and non-skid floor mats were provided. Water temperature in restrooms were measured to be between 105.8 and 109.7 degrees Fahrenheit.
Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked in a kitchen cabinet, inaccessible to clients in care. Fire extinguisher was charged, mounted and located in the kitchen. Continued on LIC 809-C.
