Facility Evaluation Report
On May 29, 2026, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of conducting the required annual inspection. LPA explained reason for visit and was greeted and granted entry by receptionist. During the visit, staff on duty contacted the Director Bob Henning about the visit. Bob arrived shortly to assist with the inspection.
LPA toured the interior and exterior portions of the facility with Director. The facility is a two-story structure and is licensed for 110 non-ambulatory clients. For this visit, there are a total of 100 clients in attendance.
LPA observed that the facility has designated areas for different activities, including various activity rooms. LPA observed clients participating in activities supervised by staff. LPA observed clients engaging in exercise, arts and crafts, and reading. The facility has two private changing areas for client use, two restrooms on the first floor and four restrooms on the second floor. Restrooms were observed to be in good repair. Water temperature in restrooms was measured to be 112.4 degrees Fahrenheit. LPA did not observe any items accessible to clients which would pose a risk to their health and safety.
Fire extinguisher was charged and located in the hallway. Fire extinguishers were dated and tagged on May 27, 2026. LPA observed that the most recent fire inspection conducted at the facility was held on January 13, 2026 in which the smoke detectors were tested. LPA observed all the required postings in the reception area of the facility.
Facility had back-up emergency food and water supply, locked in a storage room. LPA observed that First Aid Kit had all the required components. Medications were observed to be locked in the nurse’s station. Toxins were also observed to be locked and inaccessible to residents in care. LPA observed 3 evacuation chairs by the stairs and elevator. Continued on LIC-908C.
