Facility Evaluation Report

On September 9, 2025, Licensing Program Analysts (LPAs) Brandon Lopez and Garlli Tat made an unannounced Case Management visit. LPAs were greeted and granted entry into the facility by staff after explaining the purpose for the visit. Program Manager (PM) Ryan VanderYacht was present and assisted on today's inspection.

LPAs are conducting a Case Management visit to follow up on self reported Usual Incident/Injury Report (UIIR) that was received by the Regional Office on September 3, 2025. Per the UIIR, Client #1 (C1) passed away at the hospital on August 31, 2025. On today's visit, LPAs conducted a tour of the physical plant to conduct a health and safety check. LPAs observed clients to be participating in various activities. No health or safety concerns were observed during the visit. LPAs also reviewed and obtained relevant documents to the incident which include the staff roster, client roster, and C1's records.

Based on observations made during today’s inspection, no deficiencies are being cited per Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with Program Manager Ryan VanderYacht and a copy of the report was provided.
