Facility Evaluation Report

Licensing Program Analyst (LPAs) Kimberly Lyman and Fred Arias conducted an unannounced Plan of Correction visit. Upon arrival, LPAs observed facility is tented for termites. LPA contacted Administrator by telephone to determine the situation. Administrator Mark Cruz stated facility will be tented until tomorrow, December 7, 2024 and residents had been relocated. Mark stated advising LPA Cho of the circumstances. LPA advised would follow up with visit once the extermination was completed.
