SOUTHERN CALIFORNIA SUNRISE RECOVERY CENTER

Facility did not ensure admission agreement was signed by client or authorized representative.

Licensing Program Analyst (LPA) Rose Ruppert made an unannounced visit to the facility to amend language in the LIC 9099-D. LPA was greeted and granted entry by behavioral therapist and met with Andrew Crawford, Program Director.

There are currently no clients in care at this location. LPA explained the purpose of the visit and had Program Director sign amended reports. An exit interview was conducted with Andrew Crawford, Program Director and a copy of this report and the amended LIC 9099-D was provided to the facility.
