2025 and document was signed prior to admission on February 10, 2025. The allegation that the facility did not have written assessment of clients' needs and services plan is unsubstantiated.

LPA and PD reviewed three of three client records (C1, C2 and C3). The Treatment Plan for C1, admitted on March 1, 2025, documented client's achievement of goals which was signed dated on March 3, 2025. The allegation that facility did not have treatment plan to evaluate clients' achievement of goals is unsubstantiated.

LPA and PD reviewed discharge plans for Client #4 (C4) and Client #5 (C5). Both clients were discharged to Partial Hospitalization Programs (PHP) which provide all follow-up services and are scheduled by the PHP program. The PHPs are identified on the discharge documentation and it was advised for facility to document that PHP provides these services for clarification. The allegation facility did not follow their own discharge practices is unsubstantiated.

Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are unsubstantiated. Based on the observations and file reviews made during today’s visit, the facility appears to be in compliance with Title 22 Division 6 of the California Code of Regulations. An exit interview was conducted with Andrew Crawford, PD and a copy of the report was given at the time of the visit.
