Facility Evaluation Report
On February 13, 2026, Licensing Program Analyst (LPA) Garlli Tat conducted an unannounced visit to the facility for the purpose of conducting a required annual inspection. LPA explained reason for visit and was greeted and granted entry by staff on duty. During the visit, staff contacted facility administrator (AD) Michael Villareal about the visit. Program Director Andrew Crawford arrived shortly and assisted with the visit. For this visit, there is one staff member on duty. He is background cleared and associated.
LPA toured the interior and exterior portions of the facility with Andrew. The facility is a single level structure licensed for six clients aged 18 and above. The facility is a Social Rehabilitation Facility (SRF) licensed for six ambulatory clients. For this visit, there is currently one client in care.
There are a total of four client bedrooms, two are private client rooms, and two are shared client rooms. LPA toured each bedroom in the facility and observed that bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. Smoke and carbon monoxide detector were tested and operational. There are a total of three bathrooms. Bathrooms were observed to be in good repair, toilets were operational, and equipped with soap, toilet paper, and paper towels. Water temperature in bathrooms were measured to be between 109.5 and 111.7 degrees Fahrenheit.
Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked in the office and inaccessible to clients in care. LPA observed that the facility stored linens in the hallway. Fire extinguisher was charged, mounted and located in the kitchen. Fire extinguisher was dated and tagged on June 25, 2025.
