Facility Evaluation Report
On May 8, 2026, Licensing Program Analyst (LPA) Garlli Tat made an unannounced visit to the facility for the purpose of conducting a case management inspection. LPA is following up regarding a self-reported incident that was received by the Department on May 6, 2026 involving misconduct between client and staff. LPA was greeted and granted entry into the facility by Program Director Andrew Crawford after explaining the purpose for the visit. Administrator Michael Villareal (AD) was notified via telephone about the visit. Program Director assisted LPA with the inspection.
During the course of the visit, LPA accompanied by staff, toured the physical plant of the facility. LPA observed that the facility is currently vacant. LPA observed the facility to be clear of any hazards. No health or safety concerns were observed. LPA also collected pertinent records to the complaint such as the staff and resident records.
Due to insufficient information available at this time and interviews that need to be conducted, this incident needs further investigation. An exit interview was conducted with Program Director Andrew Crawford and a copy of the report was provided.
