Facility Evaluation Report
Licensing Program Analysts (LPAs) Celine Rodriguez and Garlli Tat conducted an unannounced required annual inspection. LPAs explained reason for visit and was greeted and granted entry by staff on duty. During the visit, LPAs met with Program Director (AD) Andrew Crawford. LPAs observed that all current staff are background cleared and associated to the facility.
LPAs toured the interior and exterior portions of the facility with AD Crawford. The facility is a single level structure and is licensed for 6 clients. For this visit, there are a total of 6 clients in care.
There are a total of 2 shared client bedrooms, and 2 client private rooms. All bedrooms were provided with furniture in good repair, clean linens, adequate storage space, and kept free of tripping hazards. There are a total of 3 bathrooms of which 2 are designated for client use, and 1 designated for staff use. Restrooms were observed to be in good repair, toilets were operational, and water temperature was measured to be at 115.9 degrees Fahrenheit.
Smoke and carbon monoxide detector were tested and operational. Facility met the minimum two-day perishable and seven-day non-perishable food supplies. Sharp items and knives were locked and inaccessible to clients in care. LPAs observed fire extinguishers throughout the facility, of which were observed to charged, mounted, and located in multiple areas of the facility and dated for 2025. LPAs observed that the facility has an emergency disaster and evacuation plan and is posted in the living room. Facility had back-up emergency food and water supply, located in the medication room. LPAs observed that the facility First Aid Kit had all the required components. Medications and toxins were in a locked cabinet located in the garage and in the living room cabinet.
